Educational Certificates: 10th Standard/SSLC Mark Sheet and Certificate 12th Standard/Intermediate Mark Sheet and Certificate (with Biology/Mathematics) Diploma in Optometry / Ophthalmic Assistant Certificate (2 years) All relevant mark sheets and provisional certificate 2. Identity Proof Documents: Aadhar Card (Original + Photocopy) PAN Card (Original + Photocopy) Voter ID Card (Original + Photocopy) Passport (if available) Driving License (if available) 3. Category/Reservation Certificates: Caste Certificate (SC/ST/OBC) - if applicable EWS Certificate with validity - if applicable Creamy Layer Certificate (for OBC candidates) Disability Certificate (for PwD candidates) - 40% and above 4. Other Mandatory Documents: CBT Exam Admit Card Application Form printout Date of Birth Certificate Recent passport-size photographs (10-15 copies) Domicile Certificate (if required for Bihar residency benefits) Important: Candidates must carry documents in a TRANSPARENT FILE FOLDER. All photocopies must be self-attested. Original certificates will be verified and returned on the same day. Important Instructions for Document Verification Candidates appearing for Bihar SHS Ophthalmic Assistant document verification must follow these important instructions: 1.
